The Program of All-Inclusive Care for the Elderly is a United States health care program that provides comprehensive health services for the elderly. CenterLight Teamcare

Lok
On Lok has added a social-day care center to its On Lok model. It also provides in-home care, home-delivered meals, and housing assistance to elderly individuals. These services are an extension of the On Lok care model, which provides basic medical care as well social support for older people. A federal grant from the Department of Health and Human Services supports the growth of On Lok, allowing it to develop a national model for all inclusive care for the elderly. The National PACE Association is established with On Lok's support and there are now 21 PACE programs operating in nine states.
PACE Southeast Michigan
The PACE Southeast Michigan program provides all-inclusive care. This includes a variety of services to assist seniors in living independently at home. These services can include physical therapy, counseling and social work. In addition, they provide prescriptions, outpatient services, and behavioral health and substance abuse care. In addition to these services, PACE Southeast Michigan also offers recreational activities, transportation, and home maintenance.
WelbeHealth’s model of care
